Castiel sleeps better after cutting. He even likes to be held; the firmness of Balthazar’s arms around him is comforting, grounding.
Naomi’s explained the medical side of it, of course. She’s told him all about endorphins, “feel good” hormones, dissociation and natural highs.
Castiel isn’t particularly interested, though. What matters is that he can sleep, concentrate, leave his room again. What matters is that he can dream properly, plan more than a day ahead.
What matters is that it feels good again when Balthazar smiles and holds him close.
